Eligibility for Laboratory Certification
A laboratory is eligible to be considered for drinking water certification by the State of Ohio only if it is either located within the state boundaries or is owned or operated by a public water supply within the state boundaries.
A certificate of approval to perform drinking water analyses is issued by the Ohio EPA, Division of Drinking and Ground Waters (DDAGW) to a laboratory achieving a satisfactory evaluation based on an on-site survey.
Period of Validity
The maximum length of time a certificate remains valid is one year from the date of issuance for an initial certification and three years from the date of issuance for a renewal of certification. To renew a certificate of approval, another on-site survey must be performed.
Obtaining an On-Site Survey
To be eligible to obtain an on-site survey, a laboratory must have received an approval letter from the Ohio EPA, Division of Environmental Services (DES), Laboratory Certification Section, for the current laboratory floor plans, have participated acceptably in any required Proficiency Test, and have submitted an application for an on-site survey. If the submitted survey application is acceptable, an on-site survey will be performed according to the date scheduled by the Laboratory Certification Office.
Approval for Laboratory Floor Plans
Laboratory floor plans which have not been formally approved must be submitted for review and approval to the attention of Mr. James W. Evans at following mailing address:

An informational 26 item checklist used in the plan review process can be accessed from this webpage or will be mailed to the laboratory upon request. After approval of the laboratory plans has been obtained, revised laboratory plans must be submitted prior to any proposed laboratory remodeling or relocation.
Participation in Proficiency Testing (PT)
Participation in PT is required for a laboratory which has or is applying for drinking water certification to perform analyses involving Maximum Contaminant Level (MCL) standards. A laboratory can arrange to participate in PT and request information on acceptance criteria by contacting the DES Laboratory Certification Office at (614) 644-4245.
Obtaining a Survey Application Form
A survey application form may be accessed from this site. Separate application forms are available according to the type of testing: Microbiological, Wet Chemistry, Trace Metal, Trihalomethane (THM)/ Volatile Organic Chemical (VOC), Pesticide/ Synthetic Organic Chemical (SOC), and Radiological. Contact the DES Laboratory Certification Office at (614) 644-4245 to obtain any of these applications by mail.
Submittal and Acceptance of the Survey Application
To be accepted, the completed application form should be accompanied by the following items:
- A copy of the approval letter for the laboratory floor plans;
- Documentation of acceptable PT results or training:
- Chemical Application - Documentation of acceptable PT results if certification for maximum contaminant level (MCL) analyses is requested. A copy of the evaluation for the most recent PT must be provided for initial certification. A copy of the evaluation for the two most recent PTs must be provided for renewal of certification which indicates acceptable performance for at least one of the PTs.
- Microbiological Application - Documentation of acceptable PT results. Documentation showing successful completion of the Operator Training Microbiological Water Testing course or equivalent for a laboratory which has not been previously certified or has lost all previously approved analysts.
Submittal of Survey Fee
The following fees will be charged for services for the evaluation and certification of laboratories and laboratory personnel for determining the qualitative characteristics of drinking water. The fee for these services must be submitted within 30 days from the acceptance of an application . An individual laboratory will not be assessed a fee more than once during a certification (three year) period, unless the laboratory requests the addition of analytical methods or analysts, in which case the laboratory must pay $1,800 for each additional survey requested.
